Physicians and providers at King's Daughters' Health encompass a wide range of specialties. Our goal is to offer you exceptional care close to where you live and work. We have provider offices in Madison, Hanover, Versailles, and Vevay (Indiana), along with Carrollton and Milton (Kentucky).

Tele-Health (Virtual Visits)

Many KDH providers are now providing temporary Tele-Health (virtual visits) for patients. To learn more or to schedule an appointment, contact your provider's office.

New patients

If you need help finding a primary care physician/provider, call our New Patient Access Line: 812.801.8995 or complete the New Patient Request Form below. A provider specialist will be happy to assist you. Note: While our goal is to follow-up on messages and form requests within a few hours, there are times it may take longer while we assist others. Forms submitted after business hours or on weekends will be followed-up on the next business day (Monday - Friday 8:00 a.m. - 4:30 p.m.)
