Welcome to the MyKDH Patient Portal. Using the portal, you have access to your personal health information via a computer, laptop, smartphone, or tablet. You may also message your physician (provider) and request an appointment. If you have not set up a MyKDH account, we can help. Contact Health Information Management at (812) 801-0520.

What is MyKDH

MyKDH is an online patient portal tool that helps you manage your healthcare information easily and securely. The MyKDH web portal is available 24 hours a day, 7 days a week. It gives you the option to access your health information online or from your smartphone or tablet.

What information is available in the portal?

Beyond a health summary, a variety of health information is available via the MyKDH Patient Portal. This information includes:

  • Laboratory results (under “Results”).
  • Radiology reports (under “Reports”).
  • Visit history.
  • Discharge instructions.
  • Allergies and conditions.
  • Medications with instructions.
  • Messaging with your physician (provider).
  • Request an appointment with your physician (provider).
  • Upcoming appointments (scheduled appointments).

Please note: It may take up to four business days for information such as radiology reports or laboratory results to show up in MyKDH patient portal. Talk to your personal provider if you have specific questions about test or lab results. Certain reports and results may not be available at this time on the portal.

Where does my health information come from?

All of the information in the MyKDH Patient Portal comes from your KDH Electronic Health Record (EHR). This ensures that you have access to the most accurate, up-to-date information.

What information do I need to enroll in MyKDH?

You must have an email address on file with us. When you register for any services at King’s Daughters’ Health (inpatient or outpatient), give the registration clerk the valid email address that you would like to use. During the self-enrollment process, you will be asked for the following:

  • Name
  • Date of birth
  • Email address (it must match what we have on file)
  • Medical record number (this can be provided to you during your visit) … OR … Last four (4) digits of your Social Security Number.

Can my family have access my information on MyKDH?

You may give family members, such as parents or healthcare proxies, access to your portal information. This requires a written consent from you and you will need to visit one of our Norton KDH Health Information Management offices – Downtown Medical Building (1st Floor), 630 North Broadway, Madison or Main Campus Medical Building, 1373 East SR 62, Madison (Medical Office Entrance – 1st Floor) to complete the paperwork. Parents may register as proxies for their minor children.
Note: Once a dependent turns 18, information about the individual is no longer accessible to parents without authorization from the child.

How do you keep my information safe?

MyKDH passwords are encrypted, and URLs are rewritten so that they cannot be copied and pasted. You and your authorized family members are the only ones who can access your MyKDH portal information. There is also a timeout feature built into the software. If you accidentally leave your portal page open, it logs you out after a short period of time without activity.
