2022 Community Health Needs Assessment surveys now available
Every three years, Norton King's Daughters' Health completes a Community Health Needs Assessment to help identify the leading health-related issues facing individuals and families in the communities we serve.
Surveys for 2022 Community Health Needs Assessment (CHNA)
We encourage area residents to complete these important surveys. Surveys are available through July, 2022. The CHNA surveys are open to residents of Jefferson County (IN), Switzerland County (IN) and Trimble County (KY). Use the links below to fill out the survey:
