Due to the recent surge in local COVID cases, we have re-opened our COVID Clinic at the Main Campus Medical Building in Madison. If you have questions regarding COVID testing, symptoms and care, call the COVID Hotline number above. A member of our team will help answer questions and set up an appointment at the COVID Clinic, if needed.

We strongly encourage anyone needing a COVID test due to symptoms of the illness or due to exposure to COVID to schedule an appointment through the State's scheduling module - highlighted below:

Please note the updated schedule below regarding FREE COVID-19 testing through the State of Indiana. If needed, COVID-19 testing can be ordered by the nurse practitioner during a scheduled visit (in this case, a testing fee will be included).
RAPID TESTS are currently unavailable at the State of Indiana testing location inside Convenient Care. This is due to a national shortage. PCR tests are available.
