Vevay - Sports Physicals

and Hannah Lair, APN will be providing free sports physicals to junior high and high school students from 2:00 to 6:00 p.m. on Thursday, July 18 at the Switzerland County Medical Building, 1190 West Main Street, Vevay. No appointment is necessary.

This is a great opportunity to Switzerland County athletes to receive a sports physical in advance of the upcoming 2019-2020 school sports season(s). Important notes:

  • Junior High and High School Students who are eligible to play on their school team(s).
  • IHSAA / KHSAA Medical History Forms must be completed by a parent/guardian and signed prior to attending (no exceptions). Forms are available at your school or can be downloaded online.
  • Free sports physicals are intended for students currently in good health who do not have a chronic or underlying health condition - such as diabetes, heart abnormality, asthma, etc. Students with a pre-existing condition need to schedule an separate appointment and/or see their full-time provider.
  • Girls are encouraged to wear a swimsuit top or sports bra underneath their shirt when arriving for their sports physical.
