Every three years, Norton King’s Daughters’ Health completes a Community Health Needs Assessment to identify the leading health-related issues facing individuals and families in the communities we serve.
We are asking individuals who live in Jefferson or Switzerland counties in Indiana, or Trimble County in Kentucky, to complete a Community Health Needs Assessment Survey. Links for both English and Spanish versions are located below and on the drop-down menu to your left. Data from the surveys will be included in a wider assessment that will help us partner with other organizations to identify and develop plans to address some of the most significant health issues in the region.
